With this form, you can sign up as a Club with Lionsclubs.net. This is the first step towards getting your very own Lionsclubs.net Website!

If you require additional assistance, please refer to the 'Help' link in the main menu or call 519-341-3840 from 8 AM to 8 PM EST Monday through Friday.

All fields in this form are required fields, with the exception of 'FAX'. Please use the following guidelines:

Username: This will become your login username. Use up to 20 characters, alpha and/or numeric and the text is case sensitive. (i.e. 'John1234')
Password: This will become your login password. Use up to 20 characters, alpha and/or numeric and the text is case sensitive. Your password should be different than your username. (i.e. 'Jays222')
First Name: Up to 50 characters.
Last Name: Up to 50 characters.
Email Address: Up to 100 characters.
Address: Up to 100 characters.
City/Town: Up to 50 characters.
Province/State: Select from 'Drop Down' menu.
Country:Select from 'Drop Down' menu.
Postal/Zip Code: Up to 9 characters.
Phone: Up to 20 characters.
Fax: Up to 20 characters.

Click 'Add Club Website' after completing the form.

You will recieve an email to confirm your registration. Follow the email instructions to complete the registration process.

Once you have completed your registration you will have 15 days to build your website before payment is required.

Payment should be make by cheque for up to a 3 year term @ $75.00 per year plus GST =
One Year Term: $78.50
Two Year Term: $157.25
Three Year Term: $235.50
